- In the past 30 days, how often have you experienced a form of physical pain?
Mental Health Questionnaire:
- During the past 30 days, how often have you felt particularly low, down, or emotionally exhausted?
- During the past 30 days, how often has your mental health impacted the quality of your relationships?
- During the last 30 days, how often did you experience a sense of purpose or meaning in your life?
